How to Apply for the NDIS (Access Request form)

Feeling overwhelmed about accessing the NDIS? We're here to help

If you're ready to access the NDIS but don't know where to start - you're not alone. Our friendly team is here to help you understand and apply for the NDIS.

Many people experience stress, anxiety and feelings of overwhelm when applying for the NDIS. It can be difficult to explain the everyday impacts of a disease that is often changing, particularly when symptoms can be different from person to person.

How the NDIS Engagement team can help

We're here to help you understand, navigate and apply for the NDIS. We can help the NDIA understand how MS affects your everyday life, your personal goals and the support you need to achieve this.

We have helped thousands of people living with neurological conditions access the NDIS. People who get help from our team have better success accessing the NDIS and getting the plan they need first time, with the support and services they need to meet their goals.

We can also help you re-apply if your previous application has been denied.

How to access the NDIS Engagement team

Our NDIS Engagement service is provided completely free of charge. It is delivered via telehealth. Contact MS Connect on 1800 042 138 or msconnect@ms.org.au to get started.

What is an NDIS Access Request Form (ARF)?

The ARF is an application for the NDIS. You can download it from the NDIS website.

The form requires your basic information and residency evidence. You will need to provide evidence about your disability and how it impacts your everyday living.

You will need your treating health professional, such as your GP, an allied health professional, or your Neurologist to complete the application form.

There is also an option for verbal access request if you need it.
