How to access the NDIS Engagement team

What is an NDIS Access Request Form (ARF)?
The ARF is an application for the NDIS. You can download it from the NDIS website.
The form requires your basic information and residency evidence. You will need to provide evidence about your disability and how it impacts your everyday living.
You will need your treating health professional, such as your GP, an allied health professional, or your Neurologist to complete the application form.
There is also an option for verbal access request if you need it.
