The first step to access the NDIS is to complete the NDIS Access Request Form (ARF).
What is an NDIS Access Request Form (ARF)?
The ARF provides the National Disability Insurance Agency with the information they need to decide if a person is eligible to become a participant in the NDIS. The ARF is only available from the NDIS.
Think of the ARF as an application form for NDIS. The form requires your basic information and residency evidence. It may also require you to provide evidence about your disability and how it impacts your everyday living.
You will need your treating health professional, such as your GP, an allied health professional, or your Neurologist to complete the application form.
The access request form is an important step in receiving NDIS support as it is used to assess your eligibility and help streamline your application process. There is also an option for verbal access request should you require.
