MS Case Management
Our MS Care Team offers comprehensive case management services. This can be helpful if you're experiencing changes in your condition and require extra support to meet your daily needs.
Case management involves an MS team member working with you, and possibly your family/carer, GP, neurologist and other service providers, to develop and facilitate a plan of care to meet your individual needs. It often involves making referrals to other services and maintaining ongoing contact with you and your service providers to ensure that the plan of care is meeting your needs
Our MS Care service continues to provide these services in areas which are not rolling into the NDIS just yet. If you are in an NDIS rollout area, we offer MS Support Coordination.
