MS Photo ID Card
Our free MS Photo ID Card is available to all people who are registered with MS who have a confirmed diagnosis of multiple sclerosis.
The card can be useful for telling others that you have multiple sclerosis. For example, it can be used at a bank to explain why a person’s registered signature may have changed.
Sometimes it’s also useful to explain why you may have difficulty with vision, balance, coordination or slurring of speech.
How to apply for an MS Photo ID Card
To obtain an MS Photo ID Card, you need to:
- Download and complete an application form or contact MS Connect™ (Freecall 1800 042 138 or email email@example.com) and ask them to post an application to you.
- Provide a written confirmation of your diagnosis in the form of a letter or report from your doctor, or your doctor can complete the ‘confirmation of diagnosis statement’ on the form.
- Provide a current passport-style photograph showing your head and shoulders either as a printed photo or supplied electronically in JPEG format.
- Provide a copy of another form of photo ID such as a driver’s licence or passport; or the printed photograph can be signed on the reverse side by your doctor, health professional or an MS team member who will also need to complete the confirmation of identity statement on the form.
The MS Photo ID Card is processed and then mailed out to the address supplied on the application form.
